Abstract
Objective To identify childhood adverse factors influencing inflammatory bowel disease(IBD)and provide a scientific basis for screening seafarers under chronic stress. Methods Data were obtained from the UK Biobank. Study subjects were assessed for childhood adversity factors(including sexual abuse,familial rejection,delayed medical care during illness,lack of affection,and abuse)and prenatal smoking,with subsequent IBD occurrence in adulthood. Cox proportional hazards regression models were used to examine associations between childhood adversity and prenatal smoking with the risk of ulcerative colitis(UC)and Crohn's disease(CD),respectively. Results Compared with the low childhood adversity group,moderate childhood adversity group significantly increased UC risk( HR =1.4,95% CI :1.01-1.95, P =0.046),while high childhood adversity group also elevated UC risk( HR =1.7,95% CI :1.09-2.67, P =0.020). No association was found between childhood adversity and CD risk or between prenatal smoking and IBD risk(all P >0.05). Conclusion In seafarer screening,individuals with childhood adversity(e.g.,abuse,neglect,or prolonged exposure to unstable family environments)warrant focused attention for potential IBD risk.</p>
